What Is Sarcoidosis?

Sarcoidosis is a complex inflammatory disease characterized by the formation of granulomas in various organs, most commonly affecting the lungs, eyes, skin, heart, and nervous system. 

Granulomas are small collections of immune cells that form in response to inflammation. This condition can manifest with a range of symptoms, including cough, shortness of breath, skin rashes, eye irritation, heart palpitations, and neurological issues. 

Some individuals may also experience a syndrome known as Löfgren syndrome, characterized by joint pain, fever, and enlarged lymph nodes.

Diagnosing Sarcoidosis

Diagnosing sarcoidosis can be challenging due to its nonspecific symptoms and the lack of early indicators. 

Common diagnostic tests include: 

  • Blood and urine tests
  • Chest X-Rays
  • CT scans
  • Lung function tests
  • Eye exams
  • ECG Or EKG
  • PET Or MRI scans
  • Tissue biopsies

These tests help identify the presence of granulomas and assess the extent of organ involvement. Sarcoidosis is typically classified into four stages based on the severity of organ inflammation and the progression of the disease.

However, it's essential to understand that these stages do not necessarily indicate the severity of the condition, nor do individuals progress through each stage sequentially. Rather, they primarily describe the distribution of granulomas within the lungs and lymph nodes.

Individuals with sarcoidosis may experience fluctuations between stages, and in some cases, granulomas may resolve entirely. The only stage considered irreversible is stage four, characterized by pulmonary fibrosis, leading to permanent lung damage.

Stage 0 indicates that X-rays do not reveal any evidence of pulmonary sarcoidosis, with both lungs and lymph nodes appearing normal. 

Stage 1 involves the presence of granulomas exclusively within the lymph nodes. 

Stage 2 signifies the presence of granulomas in both the lymph nodes and the lungs. 

Stage 3 indicates granulomas exclusively within the lungs. 

Stage 4 is identified by X-ray evidence of pulmonary fibrosis, indicative of permanent scarring of the lungs.

Common Medications

Medications are often prescribed to manage inflammation and alleviate symptoms associated with sarcoidosis. Corticosteroids, such as prednisone, are commonly used to reduce inflammation and suppress the immune system. Methotrexate, antimalarials, TNF inhibitors, corticotropin, and other immune system suppressors may also be prescribed, depending on the severity of the condition. However, these medications may come with potential complications and side effects, including weight gain, mood changes, osteoporosis, and increased susceptibility to infections.

Surgery

In severe cases of sarcoidosis where organ damage is significant, surgical interventions may be necessary. Organ transplants may be performed for individuals with severely damaged heart, lungs, or liver. In extreme cases, pacemakers or defibrillators may be implanted to manage cardiac complications associated with sarcoidosis.
