Max Lugavere has been back in my orbit for years now, but this conversation felt different - heavier, and more urgent. He opens up about his mom's diagnosis with Lewy body dementia, the same disease Robin Williams had, and what it was like watching someone he loved lose herself in slow motion starting around age 58. That experience became the documentary Little Empty Boxes, and it's also what pushed Max to go all-in on the research behind cognitive decline - not just what causes it, but what we can actually do about it.

We spent a good chunk of this episode picking apart the amyloid plaque hypothesis, which has dominated Alzheimer's research for decades and is now facing serious scrutiny. Max's argument, laid out in his peer-reviewed paper in Missouri Medicine, is that Alzheimer's is really a midlife disease - the damage starts decades before anyone notices a symptom, which means the food-first interventions have to start decades earlier too.

From there we got into the weeds on protein - why 1.2g/kg should be the floor, not the ceiling, and why anyone lifting weights needs closer to 1.6g/kg. We talked through the MIND diet, lutein and zeaxanthin, and Martha Clare Morris's work on leafy greens. Max also walked back some of his earlier hard line on seed oils - his real concern now is total fat load, not any one oil - and made a point I haven't stopped thinking about since: a saturated fat is not a saturated fat. Lumping dairy fat in with processed meat fat obscures more than it reveals.

The part I want people to sit with most, though, is what Max described from his trip to the White House to meet with RFK Jr. and the MAHA movement. Whatever your politics, the fact that food-as-medicine ideas are now in that room at all is a genuinely new chapter, and Max gave a firsthand, nuanced account of what that meeting was actually like - not the headline version.

A few of my favorite things we cover in this episode:

  • Catching up with Max Lugavere: the years since his last appearance and how his focus has sharpened toward brain longevity
  • His mom's Lewy body dementia diagnosis around age 58 - the same disease Robin Williams had - and the years of misdiagnosis and confusion that came before it
  • "Little Empty Boxes" - the documentary Max made chronicling his mother's decline, why he made it, and where to watch it
  • Why the dominant amyloid plaque model of Alzheimer's is outdated and increasingly contested in the research community
  • Alzheimer's reframed as a midlife disease - the pathology starts decades before symptoms show up
  • Max's peer-reviewed paper, "A Food-First Prescription for Cognitive Longevity," published in Missouri Medicine
  • Protein targets for brain and body: 1.2g/kg as a baseline, climbing to 1.6g/kg (about 0.72g per pound) for anyone doing resistance training, citing researcher Stuart Phillips at McMaster
  • Why lean red meat, eggs, and whole-food protein sources matter more than the food pyramid ever gave them credit for
  • The MIND diet and the case for lutein and zeaxanthin - pulling in Martha Clare Morris's research on leafy greens and cognitive decline
  • Seed oils revisited: Max's more nuanced, updated take and where he's shifted from his earlier stance
  • The real thesis isn't seed oils in isolation - it's that most people are simply eating too much fat overall
  • "A saturated fat is not a saturated fat" - why lumping all saturated fats together misses the nuance on red meat, dairy, and LDL
  • New federal dietary guidelines: protein recommendations rising roughly 50% to 1.2g/kg, saturated fat capped around 10%, the "grass loophole" closing, and full-fat dairy making a comeback
  • Inside the White House: Max's visit with RFK Jr. and the MAHA movement, and what that meeting was actually like
  • AI as an accelerant, not a replacement - Max's horse metaphor for how AI changes discovery, plus the bigger conversation around AI existential risk and the real class-action lawsuit against Anthropic that Genius Foods is part of
