Most of us learned about sunscreen from a bottle label and about skin cancer from a PSA. Neither gave us the full picture. This conversation does.
I sat down with Dr. Dennis Gross - board-certified dermatologist, former melanoma researcher at Sloan Kettering, and one of the most rigorous scientific minds in the skincare world - and within the first few minutes I already had something I didn't know. Freckles, it turns out, are not cute. They're your skin telling you it can't produce enough pigment to protect itself - which means it's reproducing the cells that make pigment instead. That's a check engine light. Whether you've spent years in the sun or think you've been careful, that kind of reframe is useful.
We went through the full landscape: UV light as a form of radiation that causes actual DNA mutations (not to scare you - sunscreen blocks it, but it's worth knowing the mechanism), how to do a mole check yourself using the ABCD method, why even a single sunburn increases your lifetime skin cancer risk, and what the data says about the sun avoidance overcorrection - specifically the vitamin D deficiency problem that comes from avoiding all sun entirely. Dr. Gross is clear-eyed about all of it. He doesn't traffic in fear or in false reassurance. He's a data person, and every recommendation he makes comes from that.
The practical section of this conversation is genuinely useful for anyone with a skincare routine. We talked about why he always formulates with mineral sunscreen and how the chalky-white stigma became irrelevant, the SPF layering math error most people make (SPF 30 plus SPF 15 does not equal SPF 45 - it equals 22.5), and the two-step exfoliation philosophy that changed the way I think about the acids I'm using. We also got into LED light therapy, collagen-building actives, topical vitamin C versus oral, and what's coming next in skincare science. If you've ever stood in a skincare aisle and genuinely not known what you were looking at, this one will help.
A few of my favorite things we cover in this episode:
- UV light as a form of radiation - what it's doing to DNA at a molecular level and why mutations cause cancer
- The ABCD rule for moles: asymmetry, border, color, diameter - and what to actually look for at home
- Freckles aren't cute: why freckling is your skin crying for help and what it signals about past sun exposure
- Even a single sunburn increases lifetime skin cancer risk - the data is clear
- Physical vs. chemical sunscreen: why Dr. Gross always formulates with mineral, and how the chalky white look became a thing of the past
- SPF math people get wrong: layering an SPF 30 with an SPF 15 doesn't give you SPF 45 - you get 22.5
- The vitamin D catch-22: why people who avoid all sun often end up deficient, and the case for D3 supplementation
- Exfoliation done right: the two-step alpha-beta peel method that activates and then deactivates the acids - and why leaving acids on all day is not what dermatologists actually do
- LED light as a skincare "ingredient": multiple red wavelengths for collagen, blue for acne bacteria, and why FDA clearance is the only thing that matters when buying a device
- Collagen-building ingredients: topical vitamin C, retinol, niacinamide - and why one plus one equals three when the right ingredients are combined
- Why topical vitamin C delivers more to the skin than any oral dose could
- What's coming next in skincare: at-home devices, ingredient synergies, precision amino acid formulations, and epigenetic skincare
