There's a telehealth patient I keep coming back to, not because her case was unique, but because so many people are living through some version of it. She was in her forties, high-functioning, had done all the wellness things, and had been to six-plus doctors for brain fog, crushing fatigue, bloating that wouldn't quit, and anxiety that came out of nowhere. One doctor told her it was just stress. Another said it was probably her age. A third handed her an antidepressant and sent her on her way. When she came to my telehealth team, we ran comprehensive labs, not the basic ova and parasite exam most doctors order, and found multiple parasite species quietly living in her gut, inflaming her intestinal wall and sending inflammatory signals straight up her vagus nerve to her brain. She was not crazy. She was not broken. She had a passenger, and she is far from alone.

Here's why this gets missed so often: the standard stool test is microscopy, someone looking at a slide, and it can miss up to 70% of infections because parasites shed in cycles. What we use instead is PCR-based stool testing, something like a GI Map, which looks at a much broader range of parasites, protozoans, bacterial and fungal overgrowth all at once. And we don't hang our hat on just one lab or make any single organism the boogeyman; we look at the larger picture of someone's gut terrain.

The symptoms are wide-ranging and often strange. Some people describe a crawling, tickling sensation under the skin, which sounds like a horror movie because, for them, it is. But most people's symptoms are far more subtle: chronic bloating, alternating constipation and diarrhea, gas, food sitting like a brick in the stomach, and brain fog that I'd call the biggest through line I see clinically, the quiet, insidious kind where you lose your train of thought mid-sentence or read the same paragraph three times. There's also a real histamine and mast cell connection: parasites can trigger histamine release, which shows up as sinus pressure, flushing, hives, or anxiety that spikes after meals.

A March 2026 study in Nature, out of UCSF with collaborators in the US and China, mapped exactly how this happens. Specialized sensory cells in your gut lining called tuft cells detect parasite metabolites and release acetylcholine, which triggers nearby cells to release serotonin, which fires the vagus nerve, the superhighway from gut to brain, sending a sustained danger signal. A 2026 review in the Journal of Neuroinflammation found the same pattern across parasites broadly: sustained inflammation, gut dysbiosis, leaky gut, and a "leaky brain" effect on neurotransmitters. Toxoplasma gondii is the extreme version of this, directly altering dopamine pathways and linked in population studies to a 2.8-fold increase in ADHD odds, but you don't need a brain-invading parasite to have a brain problem that starts in your gut.

The part I want people to hear most carefully is this: you cannot treat a parasite in isolation from the person. Parasites thrive when your nervous system is stuck in fight-or-flight, because that slows your migrating motor complex, the gut's own "housekeeper wave" that's supposed to physically sweep debris and organisms out between meals. A sluggish nervous system lets parasites stay, and their presence keeps the nervous system sluggish right back. It's a loop, and it's why food and supplements alone so often fall short.

So what actually helps? Food first: bitter foods like arugula, dandelion greens, and gentian to stimulate stomach acid and bile flow, a 4-5 hour gap between meals to let the migrating motor complex run, small amounts of fermented foods to rebuild the microbiome, and foods like pumpkin seeds, papaya seeds, garlic, and clove that are mild, broad-spectrum antimicrobials. Cut sugar and refined carbs during a cleanse. And just as important: daily nervous system regulation isn't optional, it's infrastructure. Breathwork, cold or dark showers, humming, gargling, journaling. For anyone who wants more structured support, I built a 4-month Parasite Cleanse protocol, the reset, at drwillcole.com, with a comprehensive binder, targeted antiparasitics and antimicrobials, a biofilm disruptor, and foundational nutrients built in. You'll find it linked in the show notes.

A few of my favorite things we cover in this episode:

  • A real patient case: 6+ doctors missed multiple gut parasites behind years of brain fog and fatigue
  • Why standard stool tests miss up to 70% of parasitic infections
  • The symptom people are embarrassed to mention: crawling sensations under the skin
  • Brain fog, fatigue, and histamine issues as classic, commonly missed parasite symptoms
  • A new 2026 Nature study: how parasites hijack the gut-brain vagus nerve pathway
  • Toxoplasma gondii and the gut's direct link to mood and ADHD risk
  • Why the migrating motor complex (the gut's "housekeeper wave") breaks down with parasites
  • The right way to test: PCR stool panels (like the GI Map) over basic ova and parasite exams
  • Why Blastocystis hominis is controversial, and when it actually matters
  • Food first: bitters, fermented foods, and why you can't out-supplement a bad gut terrain
  • Nervous system regulation as the most skipped, most important part of any parasite protocol
  • Inside Dr. Will Cole's 4-month Parasite Cleanse protocol
