Twice a month, we sit down for Ask Me Anything episodes to answer your health questions from the clinic lens. This one is a perfect example of why I’m always talking about context: you can’t reduce health to a single number on a lab report. We start with a big one I see constantly in telehealth patients, high LDL and “genetic cholesterol.” If you’ve been told you need a statin but you’re already doing the “healthy lifestyle” things, this conversation will give you a more complete framework for what to look at next.
We dig into how PCOS and Hashimoto’s can impact lipid markers through insulin resistance, thyroid hormone conversion, and liver pathways, and why total cholesterol alone is a poor predictor of cardiovascular events. I break down the labs that matter more for many people, including LDL particle quality (NMR testing), triglycerides, inflammation markers, and blood sugar markers that often tell the deeper story. The goal isn’t to “treat a marker,” it’s to understand why the check-engine light is on in the first place.
From there, we go into low ferritin and pernicious anemia, and why anemia isn’t always an iron issue. We talk intrinsic factor antibodies, autoimmune gastritis, absorption problems, and why dosing iron without proper labs can backfire. We also answer a hormone question that matters: spotting after missing an estradiol patch can happen, but if you’re postmenopausal and bleeding returns, that’s not something to normalize, it’s something to rule out.
