We kicked off this Ask Me Anything with some genuinely good news: Heal Your Cells, the book I wrote with Dr. Josh Axe, just hit the New York Times bestseller list. That's years of work paying off, and it means more people getting this information, which is the whole point.

Then we got into it. Our first question came from Brittany, who has MCAS and histamine intolerance and wanted to know if peptides like BPC-157 are safe for her to try. The honest answer is nuanced: histamine intolerance is a "pseudoallergy," not a true allergy, and it's rarely the peptide itself causing a flare - it's almost always the fillers, the sourcing, or the dose. We walked through the stack we actually use clinically (BPC-157, KPV, TB-500, Thymosin Alpha-1, VIP), and I shared a stat that surprised even me: a 2025 case series found 89% of MCAS patients went into remission on a GLP-1. Andrea also made a point I want people to really sit with - sometimes the nervous system's fear response to a supplement is doing more damage than the supplement itself.

Our second question, from our own team member Caitlin, was lighter but just as important: can creatine save you from "Ozempic butt"? The real answer is that GLP-1s can cost you 15 to 40% lean muscle, not just fat, so the fix isn't a magic supplement - it's resistance training, adequate protein, and yes, creatine, working together.

But the question I want people to sit with the longest came from Michelle, who asked whether millennials - or honestly any of us alive today - are actually resilient, or just stuck in nervous system burnout. My hot take: humans have always gone through brutal stuff - World Wars, the Black Plague, the Great Depression - so the hardship itself isn't new. What's different now is that we've lost our connection to people, to nature, and to God, the things that used to help us metabolize that hardship. Andrea's story about her grandfather, who fought at Normandy and never let that struggle define him, stuck with me. We didn't used to talk about our pain this much, but we also didn't do much about it once we did. That's the real work now - not just naming what happened to us, but choosing, every day, to heal from it.

A few of my favorite things we cover in this episode:

  • Question 1 (Brittany): is it safe to try peptides like BPC-157 if you have MCAS or histamine intolerance, or will they trigger a flare?
  • Histamine intolerance explained as a "pseudoallergy" - non-IgE mediated, mast cells releasing histamine via MRGPRX2 receptor activation and not knowing how to calm back down
  • Bio-individuality framing: underlying drivers (mold, chronic Lyme, SIBO, gut dysfunction, trauma) determine someone's histamine "resilience," so there's no one-size-fits-all answer
  • It's rarely the peptide itself causing a flare - more often fillers, purity, sourcing, or dosing (start low and slow, work with a practitioner)
  • The histamine-friendly peptide stack: BPC-157, KPV, TB-500, Thymosin Alpha-1, and VIP (vasoactive intestinal peptide) for SIRS-type presentations
  • A 2025 case series in the American Journal of Medical Sciences: 47 people with MCAS put on GLP-1 receptor agonists (semaglutide) saw an 89% remission rate in their MCAS symptoms
  • Mast cell stabilizing support to pair with any of this: quercetin, luteolin, stinging nettle, DAO (diamine oxidase), and vitamin C
  • The nervous system piece: somatic breathwork, EMDR, vagus nerve stimulation devices (Pulsetto and similar auricular devices), affirmations, and the "journal speak" method for processing trauma stored in the body
  • Question 2 (Caitlin, from Dr. Cole's own patient experience team): does creatine prevent "Ozempic butt" - the fat-pad loss from GLP-1s?
  • Reality check: lean muscle mass can account for 15-40% of total weight loss on a GLP-1, not fat - so the fix is resistance training plus creatine plus adequate protein, not a single supplement
  • A study finding unilateral leg workouts spiked mitophagy and mitochondrial biogenesis only in the worked leg - why "leg day" matters more than people think for longevity
  • The team's own real-world experiment: 10 squats (then pushups) every hour at their desks, inspired by research showing it rivals an hour of walking
  • Question 3 (Michelle): are millennials (or any generation alive today) actually resilient, or are we stuck in collective nervous system dysregulation?
  • Dr. Cole's take: humans have always faced brutal hardship (World Wars, the Black Plague, the Great Depression) - the real difference today is disconnection from people, nature, and God, not the hardship itself
  • Andrea's personal story: her grandfather fought at the Battle of Normandy and never showed the struggle she now realizes he carried
  • Why the modern 24-hour replay of trauma (same 9/11 footage every year) keeps nervous systems in fight-or-flight in a way earlier generations' lived experience didn't
