I went to New York for this one, and I'm glad I did.
Dr. Joe Damiani has built one of the most practically useful platforms in the physical therapy world around something most practitioners avoid: head, neck, and jaw pain. He came to it by accident — someone at the clinic where he was working asked if anyone treated TMJ and his colleague said no because she wasn't putting her hands in anyone's mouth. He said yes. And then it just kept expanding, because if you know how to treat the jaw, patients with ear pain start showing up. Then patients with headaches. Then patients with dizziness and brain fog. The referral network is the anatomy.
What I love about how Dr. Joe thinks is how closely it aligns with functional medicine at a muscular and neurological level. The conversation he has with patients mirrors the one I have with mine: the downstream symptom is not where the root cause lives. A muscle that keeps tightening back up after you stretch it is not a muscle problem. It's the nervous system telling that muscle to stay guarded — and until you address why the nervous system is dysregulated, you'll be stretching forever. He has three buckets for what drives that dysregulation: emotions (including anticipatory fear and worry, not just past trauma), trust (the nervous system doesn't believe pain-free movement is available to it), and awareness (constantly checking in on the pain keeps sending a signal to it). Each bucket has a different strategy, and movement is the thread that runs through all of them — because movement is objective proof. You can think your way out of a lot of things, but when your body actually moves without pain, the nervous system has no argument left.
We also got into the TMJ specifics that I think a lot of people need to hear. The dental route — the appliances, the bite adjustment, the splints — has very little evidence behind it for pain resolution. The research shows there's almost no correlation between bite alignment and TMJ pain. Some people with perfect bites have chronic jaw pain. Some people with what dentists call a gnarly bite have zero jaw pain. The night guard is genuinely useful, but for protection — to keep you from grinding your teeth down — not as a treatment. And the biggest revelation that keeps coming up in this conversation: most of the jaw pain Dr. Joe sees is actually coming from the neck. If you turn your head and feel it in your jaw, that's not a jaw problem. That's a neck-to-jaw referred pain pattern, and the jaw is completely innocent.
