Three questions in this AMA. Each one is the kind that doesn't have a quick answer — and that's exactly why we spend time on them.
The first was from Ashley, whose son was a Division I collegiate wrestler. On the first day of college practice, the coach handed out standing Valtrex prescriptions to the entire team and told them it was only a matter of time before they contracted herpes gladiatorum from the mats. Sure enough, he did. Now he's graduated, out of that environment, and wants to know whether he's stuck with this virus — and whether there's a functional approach that goes beyond suppression. What I love about this question is how many people have some version of it. Herpes gladiatorum, Epstein-Barr, shingles, cytomegalovirus — these are all in the same viral family, all managed the same way by conventional medicine, and all significantly influenced by something conventional medicine almost never addresses: the terrain of the body. The lysine/arginine food framework, L-lysine supplementation at therapeutic doses, zinc, vitamin C, and — for those who have access — Thymosin Alpha-1, which has documented antiviral and antimicrobial effectiveness. But above all: sleep, gut health, stress regulation. Viruses don't activate in a vacuum.
The second was from Lauren, who has been in eating disorder recovery since 2018, is fully weight restored, has worked with a nurse practitioner in functional medicine, tried Vitex and inositol, and was even put on BHRT to try to kickstart her cycle. Nothing worked. And here's what I want her and everyone listening to understand: when HRT doesn't work, that's not a failed HRT trial — it's a nervous system signal. Your body will not produce its own estrogen and progesterone until it feels safe. And it doesn't feel safe from a pill. It feels safe from a regulated nervous system. The acetylcarnitine conversation is genuinely useful here — it crosses the blood-brain barrier and stimulates gonadotropin-releasing hormone, which then triggers the entire HPO axis cascade. But the equally important message is this: hormones can take upwards of two years to regulate after a period of restriction. Give it time, give your body safety, and nourish yourself toward it.
The third was from Leanne, 18 years old, who's been dealing with dizziness, lightheadedness, and vertigo-like symptoms for two years — starting after an osteopath visit. She's been to multiple practitioners with no answers. Chiropractic has helped. She suspects nervous system involvement. She's right. And the fact that nobody's given her a clear cause yet just means nobody's asked the right questions. How close is your Wi-Fi router to where you sleep? Have you had any mold exposure? Any head trauma? Any significant life stressors or viral illnesses before this started? These are the questions that open the door — and in our experience, there's always an upstream driver if you look carefully enough.
A few of my favorite things we cover in this episode:
- Herpes gladiatorum (gladiator herpes) in Division I wrestlers: why teams prophylactically prescribe Valtrex — and what to do after graduation when you have the virus but want a holistic alternative
- Herpes, shingles, EBV, CMV, chickenpox — all in the same family: why some people have viruses that never activate and others deal with constant reactivation, and what the HLA gene variant explains
- The lysine/arginine food protocol: why increasing lysine-rich foods (chicken, turkey, beans) and decreasing arginine-rich foods (nuts, seeds, chocolate) creates an inhospitable terrain for viral replication
- L-lysine 1–3g/day as the clinical research dose — below 1g is not effective for active viral issues
- Thymosin Alpha-1 as the peptide with documented antimicrobial and antiviral effectiveness
- Amenorrhea after eating disorder recovery: why the weight being restored doesn't automatically mean the period returns, and why the body needs to "trust" the nervous system before it will produce estrogen and progesterone
- Why BHRT didn't work for Lauren: when HRT doesn't kickstart a natural cycle, it's a nervous system issue — not a hormone issue
- Acetylcarnitine for hypothalamic stimulation: how it passes the blood-brain barrier and stimulates gonadotropin-releasing hormone to initiate the HPO axis cascade
- Blood sugar stability as a prerequisite for hormonal recovery in eating disorder patients — "You want to make your body not feel safe? Have imbalanced blood sugar"
- The gut as the site where hormones are converted, stored, and produced — why a disrupted microbiome from restriction is a critical missing piece
- Dizziness, lightheadedness, and vertigo at 18: the differential diagnosis from POTS and dysautonomia to neurogenic, vascular, and mold-driven causes — and why a proper workup beats guessing
- Sodium and electrolytes as the first low-hanging fruit for dizziness — requirements are 2–3x the government recommendations for many people with dysautonomia-type presentations
- Vagal nerve stimulation (auricular devices, neck stim, breathwork, humming, gargling) as the top clinical tool for nervous system dysregulation driving dizziness
- The questions nobody else asks: EMF proximity to the bedroom, mercury thermometer exposure, RAID spray as a child, electric car — why the right questions unlock the upstream driver
